A birth certificate is the ticket to various benefits a rightful citizen is entitled to. All records dating September 26, 2009 and below were lost during the flood. Registration of Birth, Foundling, Marriage & Death REGISTRATION REQUIREMENTS
ISSUANCE OF CERTIFIED COPIES AND TRANSCRIPT OF REGISTERED BIRTH, FOUNDLING, MARRIAGE. DEATH & OTHER RELATED CERTIFICATIONS |
- Request Slip
- Authorization letter in case of birth records If requesting party is not the concerned person, nearest kin or court/public official must be present in official proceedings
REQUIREMENTS FOR DELAYED REGISTRATION
(Registration beyond 30 days from date of occurrence of birth and death and beyond 15 days from date of solemnization of marriage is considered delayed registration.)
Fee for delayed registration - PhP 100.00
- Delayed Registration of Marriage
- Affidavit of delayed registration executed by the solemnizing officer or the person reporting or presenting the Certificate of Marriage
- Certified copy of the application for marriage license bearing the date of issuance of the marriage license - Delayed Registration of Death
- Affidavit of Delayed Registration executed by the hospital or clinic administrator if the person died in the hospital, clinic or similar institution; or if the person died elsewhere, by the attendant-at-death. If defaulted by the hospital or clinic administrator or attendant-at-death, the affidavit shall be executed by any of the nearest relatives of the deceased, or by any person having legal charge of the deceased when he was still alive
- Authenticated copy of the certificate of burial, cremation, or any other means of corpse disposal
Marriage License Application & Issuance
REQUIREMENTS
- Physical presence of applicants
- Birth or baptismal certificate (original and photocopy)
- 2"x 2" I. D. Picture (1 for each applicant)
- Parents’ Advice for applicants ages 21 to 25
- Father’s Consent for applicant ages 18 to 21
- Death Certificate of spouse if applicant is widowed
- Court Decision and Certificate of Finality if former marriage was void
- Copy of the Decree of Divorce or Divorce Certificate, if divorced;
- Payment of the following fees:
- Application Fee
- Marriage License
- Marriage Contract Form - Additional Requirements for Foreign Applicants:
- Legal Capacity to Contract Marriage or Affidavit of No Legal Impediment to Contract Marriage issued (original and 4 photocopies)
- Valid passport
- two (2) copies of page where the picture of the applicant and the page where the date of arrival of the same appears
- Legal Capacity to Contract Marriage or Affidavit of No Legal Impediment to Contract Marriage issued (original and 4 photocopies)
PROCEDURE:
- Submit all the required documents
- Fill up the following forms :
- Information Sheet
- Application for Marriage License, two (2) copies
- Statement that Advice of Parents or Guardian has been requested (for applicants aged 21 and under 25 years)
- Information Sheet
- Submit the forms and get Charge Slip
- Pay for Marriage Application Fee and Marriage Certificate form at the City Treasurer's Office and present Official Receipt to LCR
- Official Receipt will be returned to the applicant with the following information written at the back :
Date of Release of Marriage License (10 days after) and schedule of Family Planning & Marriage Counseling Seminar
- Attend the Family Planning & Marriage Counseling Seminar held every Friday, 12:30 PM at the Court Room
- Submit your Marriage License and all the documents attached to it to the Church/ Court or Office of the Mayor where you intend to be married.
Registration of Court Decrees/Orders
- Adoption
- Annulment of Marriage
- Correction of Entry or Change of Name
REQUIREMENTS:
- Four (4) certified true/photo copies of Court Decision
- Four (4) certified true/photo copies of Certificate of Finality
- Four (4) photocopies of subject civil document (ex. Birth Certificate)
- Payment of PhP 100.00 at the City Treasurer's Office
PROCEDURE:
- The Clerk of Court or successful petitioner shall submit to the LCR where the Court is functioning, within ten (10) days after the decree/order has become final the required documents
- LCR shall record the decision in the Registry of Court Decrees & Orders and shall assign a registry number
- Requesting party shall receive on scheduled date of release the registrant’s copy of properly marked/ stamped and annotated documents and Certificate of Registration
Registration of Legal Instruments
- Acknowledgment / Admission of Paternity
- Certificate of Legal Capacity and Legitimation
REQUIREMENTS:
- Four (4) copies of legal instruments
- Pay PhP 100.00 fee at the the City Treasurer's Office
PROCEDURE :
- LCR shall record the legal instrument in the Registry of Legal Instruments and shall assign a registry number
- LCR shall provide the registrant a copy of the document within five (5) days from the date of filing
Permit for Cadaver Disposition
- Burial Permit
- Cremation
- Exhumation of Cadaver
PROCEDURES :
- Submit the required documents to LCR and receive a Charge Slip
- Pay the fees at the City Treasurer’s Office
- Present the Official Receipt of payment to LCR
- Receive from LCR a copy of the stamped and signed Death Certificate and the Official Receipt. These shall serve as the Cadaver Disposition Permit.
