The MCHO is responsible for providing healthcare services to the Marikina citizenry, particularly the poor and vulnerable groups in the community so that they can contribute actively, productively and participate meaningfully in community life and practice healthy lifestyles.

SERVICES, REQUIREMENTS & PROCEDURES:

• FREE OUTPATIENT CONSULTATION
  
  
  1. Go to your Barangay Health Center. Open from Monday- Friday, 8:00 AM - 4:00 PM, except on holidays
  2. Bring any valid ID/document indicating residency
• SECURING A HEALTH/ MEDICAL CERTIFICATE

  Go to the Marikina City Health Office from Monday-Friday, 8:00 AM - 4:00 PM, except on holidays and bring the following:

  1. 1" x 1" picture for Health Certificate
  2. Latest Community Tax Certificate (cedula)
  3. Urine & stool sample placed in a small and clean bottle
  4. Health Certificate Fee of PhP 50.00 paid in advance

  1st day 8:00 - 10:00 AM

  WINDOW 1: Secure a number and submit the following:

  - 1" x 1" picture for Health Certificate.
  - Latest Community Tax Certificate (cedula)

  WINDOW 2: Pay PhP50.00 Health Certificate Fee

  WINDOW 3: Submit urine and stool samples

  2nd day 1:00 PM

  1. Attend the Food Handler's Seminar
     
  2. Health Certificate, Food Handler's Certificate and laboratory tests will be issued after the seminar

  Submit the following :

  FOOD BUSINESS ESTABLISHMENT

  1. Health Certificate
     
  2. Vermin Abatement Program (Pest Control)
     
  3. Inspection of Food Establishment (Sanitary Order)
     
  4. Sanitary Permit Fee - PhP 150.00

  NON-FOOD BUSINESS ESTABLISHMENT

  1. Vermin Abatement Program (Pest Control)
     
  2. Inspection of Business Establishment
     
  3. Sanitary Permit Fee - P150.00
